Maps and Directions
We realize that one of the first things most people think about when they are admitted to the hospital is going home. Getting you well enough to go home is our goal, too. When it’s time to go, please remember to make sure you have all of your personal belongings and to tell your nurse if you need help arranging transportation.
Our team is with you through every step of the healing process. To ensure that you receive seamless care even after your discharge from the hospital, our social worker/discharge planner may work with your nurse and doctor to arrange continuing care services such as home health care, skilled nursing or hospice care.
Time to Go Home
On the day of your discharge, please have your family member or the person who is picking you up arrive by 10 a.m. and report to your room. Your schedule for the day might include:
- Arise early
- Take some pain medication
- Eat breakfast
- Physical therapy
Instructions and information upon discharge could include:
- Wound care, suture removal
- Special activity precautions
- Exercise program or rehab
- Driving precautions
- Physician follow up
For those patients that will need extended inpatient rehabilitation, or care after surgery, your care team will coordinate arrangements for your transfer to a facility.
A partnership with The Pharmacy Counter provides the convenience of having your medications delivered to your home after discharge. Please contact your nurse navigator if you would like to take advantage of this service or to discuss other home medication options.
