Community Health Needs Assessments (CHNA)
A community health needs assessment (CHNA) is a formal process of collecting and analyzing health data and then assessing the needs in a defined community based on these data.
ProMedica hospitals have participated in formal county CHNA processes since 1999 and have now expanded the process to include increased collaboration with community organizations to collectively improve the health of the communities we serve. All hospitals are required to conduct a CHNA at least once every three years and adopt an implementation strategy to meet the community health needs identified through the CHNA. The CHNA takes into account input from persons who represent the broad interests of the community served by the hospital facility, including those with special knowledge of or expertise in public health. The CHNA must also be made widely available to the public, so we have provided links below to the CHNA documents for each of our hospitals. A hard copy of these documents may also be obtained by emailing your request to: firstname.lastname@example.org.
